​At ACT, we’re proud to carry on a tradition of care in the heart of Addison County. Our clinic opened within the last two years, taking root in a space with deep community ties. We were honored to step into this role after the long-standing provider, Long Trail on the Falls, entrusted us with continuing the physical therapy services they so faithfully offered for years.

Our team of local clinicians brings both clinical expertise and a genuine connection to the community we serve. As residents ourselves, we understand the unique needs of our neighbors and are committed to delivering compassionate, personalized care to help you move better, feel stronger, and live well.

Addison County Therapies is owned and operated by Addison County Home Health & Hospice, Inc. Addison County Home Health & Hospice, Inc., is a 501(c)3 nonprofit agency governed by a volunteer board of directors and certified by Medicare to provide comprehensive home care services in Addison County, Vermont. We have expanded our services to include clinic based Physical and Speech Therapy via Addison County Therapies in order to better meet community needs.

Meet physical therapist and supervisor, Jameson Sylvia, PT, DPT. Since graduating from Springfield College in 2011 Jameson has worked in a variety of settings all over the country gaining invaluable experience on treating conditions including chronic pain and dysfunction. He has since settled back in New England with his family and has enjoyed getting to know the residents of Addison County. Jameson has been trained in methods including Postural Restoration, McKenzie, and NAIOMT and works toward tailoring a treatment plan that addresses each patient’s specific goals.

Meet physical therapist Mary-Kate Mansfield, PT, DPT. Mary-Kate grew up in Addison County and obtained her Doctorate of Physical Therapy from Ithaca College. Mary-Kate is joyful to be back in Addison County with her husband and dog, serving her home community!
​Mary-Kate has trained in a variety of fields of physical therapy, including outpatient vestibular, neurologic, orthopedic, sports medicine, and aquatic therapy settings.  Mary-Kate specializes in vestibular physical therapy, utilizing our infrared video Frenzel goggles for high quality diagnosis and treatment of vertigo and dizziness issues. Mary-Kate is certified in the LSVT BIG program, which is treatment specific to working with individuals with Parkinson's disease. She loves to work with balance, fall prevention, and orthopedic conditions.

​Meet physical therapist,  Michael Cooper, PT, DPT. Michael attended the University of Northern Iowa where he received a B.A. in Movement and Exercise Science while competing on the track and cross country teams. This is where his interest in injuries and rehab started. He went on to receive his Doctorate of Physical Therapy from Clarkson University in 2015. Since then he has been working as a physical therapist in Addison County and calls Monkton his home. He has clinical interest in total joint replacements, post operative rehab and tendon injuries, but enjoys working with a wide range of orthopedic conditions to help keep people active and doing what they love. Outside of work, he enjoys spending time hiking, camping, and fishing with his partner Megan and their two dogs.

Meet speech therapist, Celina Fuller, SLP. Celina has joined us as a full-time speech therapist working in the home health and outpatient settings for Addison County Therapies and Addison County Home Health and Hospice. She has worked in acute rehab, acute care, outpatient and home health settings in Vermont since graduating from UVM in 2009. She treats adult patients with speech, language, cognitive, voice and swallowing impairments associated with conditions such as stroke, TBI, dementia, Parkinson’s disease, ALS, and head and neck cancer. She is a certified LSVT LOUD clinician, which is an intensive outpatient treatment to improve the speech of individuals with Parkinson's disease.
