About US
At ACT, we’re proud to carry on a tradition of care in the heart of Addison County. Our clinic opened within the last two years, taking root in a space with deep community ties. We were honored to step into this role after the long-standing provider, Long Trail on the Falls, entrusted us with continuing the physical therapy services they so faithfully offered for years.
Our team of local clinicians brings both clinical expertise and a genuine connection to the community we serve. As residents ourselves, we understand the unique needs of our neighbors and are committed to delivering compassionate, personalized care to help you move better, feel stronger, and live well.
Whether you’re recovering from surgery, managing chronic pain, or working toward new physical goals, we’re here to support you every step of the way — with the same spirit of service that’s been part of this location for years.
We’re grateful for the trust the community has placed in us and look forward to continuing to serve the people of Addison County for many years to come.
